Appointment Booking

Pre-Appointment

After scheduling, you will receive an email confirming your appointment as well as any paperwork that may need to be completed. If you don’t prefer to fill it out online, please arrive to your appointment 10-15 minutes early.

Please bring a photo ID, any insurance cards, and a form of payment (if applicable) to your visit. If you are under 18, you must have a guardian present at your initial visit to sign consent forms.

Cancellation Policies

Because we understand life happens, WFC does not charge late or cancellation fees. We kindly ask that you please let us know at least 2 hours before your appointment if you will no longer be able to make it. If you are more than 5 minutes late, we reserve the right to cancel or reschedule your appointment, as services may no longer be able to be provided in the remaining time allotted.

Payment Policies

We accept cash, check, debit, and credit card payments. Your patient responsibility, which will always be disclosed to you in the form of a Good Faith Estimate before services are provided, are expected to be paid at the time of service unless otherwise discussed with Dr. Jackie.
